A North American mining company faced a critical challenge—an alarming decline in safety performance, paired with a rise in lost-time injuries. A deeper analysis revealed a troubling reality: safety culture was at risk, and existing protocols, tools, and processes were falling short of operational standards. Then, the unthinkable happened—a fatal incident underscored the urgent need for transformation.

With safety performance declining, the organization sought to uncover the root causes, mitigate risk, and create a companywide culture of safety excellence. Faced with the urgent need for transformation, leadership turned to dss+ as their trusted partner to drive meaningful change.

dss+ Approach

dss+ first conducted a safety assessment to establish a risk baseline, including a strategic safety perception survey to capture employee insights on the existing safety culture. Leveraging data-driven analysis, dss+ led in-depth site evaluations and SIF/high-potential incident assessments, uncovering key opportunities to enhance workplace safety and drive lasting impact.

To make the learning sustainable, dss+ embedded themselves onsite within teams across multiple operations. Creating an enhancement to the Health and Safety Governance workstream and implementing structured on-the-job training became the foundation of Safety Leadership Development.
